About Our Doctors

  Annie Pham, D.D.S. 

Dr. Pham is proud to have provided excellent dental care for patients in Santa Clara and surrounding areas since year 2000.  Patients describe Dr. Pham as a different kind of dentist - caring, gentle and patient. She takes the time to provide you with the best care possible. Rather than being rushed, you know that Dr. Pham is there to serve your needs.

Specializing exclusively in modern dentistry, Dr. Pham uses the most advanced  materials and technologies available to protect and restore your smile.

The minute you walk through the door, you know you’ve come to the right office. Everything from soothing water fountain to digital X-ray and intra-oral camera, is designed specifically with patients’ comfort, health, and convenience in mind.

Dr. Pham holds a B.S. in Biological Sciences and a Doctorate of Dental Surgery (D.D.S) from the prestigious University of the Pacific, School of Dentistry. She continually dedicated to study advanced, post doctoral dentistry with some of the most progressive clinical dentists in the nation. Dr. Pham is currently one of the only few certified Invisalign doctors in city of Santa Clara.

Dr. Pham and her husband are members of South Bay Church, and are actively involved in various community and volunteer organizations. 

 Annie Chou, D.D.S. 

Dr. Chou was born in Taiwan but grew up in Southern California.  After graduating from the University of California, San Diego with a degree in Biochemistry and Cell Biology, and working in micro-biology research, Dr. Chou went on to earn her Doctorate of Dental Surgery from the University of California, San Francisco.  Dr. Chou’s passion is to provide high quality dental care to her patients.  She in particular enjoys working with children with whom she will quickly develop rapport and trust.  Dr. Chou strives to create a comfortable, friendly and relaxing dental experience and environment for patients through understanding and meeting patient’s individualized needs. 

During her spare time, Dr. Chou is also a passionate baker, always testing new recipes to perfect her baked goods.   She loves to do crafts, go biking, hiking with her rescue dog “Bonus”, traveling around the world, and watching movies.

Dr. Chou currently resides in San Francisco, and looks forward to working with you.

llen N. Le, D.M.D.

Dr. Le, originally from San Jose, CA, received his DMD from the University of Nevada, Las Vegas, School of Dental Medicine. After practicing for a number of years in Las Vegas, he decided to move back to California to be closer to family. After meeting Dr. Annie Pham and her husband, Tim Dang, Dr. Le chose to practice here at Perfect Smile Dental because of a shared belief in providing state of the art, relationship-based, comprehensive care that is tailored to fit into the lives and budgets of his patients.

Dr. Le is currently a member of the American Dental Association, the Academy of General Dentistry, the California Dental Association, and the Santa Clara Dental Society. Committed to providing the best care possible for his patients, Dr. Le completes over thirty hours of continuing education per year, and has taken postdoctoral courses in prosthodontics, endodontics, oral surgery, and is a member of the Mid Peninsula Progressive Dental Study Club. He is in the office Monday through Friday, including every other Saturday.

He loves spending time with his family and dog, Benji.  His hobbies include bicycling, traveling, water sports, tennis, and football.

​Dr. Heidi Lee 

Dr. Heidi Lee received her Doctor of Dental Surgery degree, with 
distinction, from the University of Western Ontario in Canada in 2007. 
After graduation from dental school, Dr. Lee enriched her clinical skills 
and dental expertise by attending a one-year Advanced Education in General 
Dentistry program at the University of the Pacific. Her quality of care, 
professionalism towards patients, and enthusiasm towards continuing 
education acquired her the Resident of the Year Award. After her residency, 
she was invited to stay on staff as a clinical faculty member to provide 
advice in treatment planning and guidance in complex restorative cases. 

Since the completion of her residency training in 2008, Dr. Lee has been 
practicing in the Bay Area. She is an active member of the Academy of 
General Dentistry, the American Dental Association, the California Dental 
Association, and the Santa Clara County Dental Association. 

Dr. Lee’s friendly and caring personality often puts her patients at ease 
in the dental chair. She works to build a rapport and long-term dental 
relationships with her patients, with the focus of ensuring patient comfort 
and life-long oral health. She offers oral conscious sedation to ensure 
patients’ comfort during treatment. 

During her down time, Dr. Lee enjoys cooking, sewing and spending time with 
her husband and two children.
